* In person: Blank forms are supplied during registration or you may print out the form below and bring it with you on City Council night no later than 6:30 p.m. in the lobby of City Hall.
* Via Fax: You may also print out and fax your form to 410-939-5920 no later than 6:30 p.m. on City Council night.
* Via Email: You may also email the form no later than 6:30 p.m. on City Council night.
